Admission Information for SageWalk, a Program for Troubled Teen Youth, Children with ADHD and Other Behavior Problems
Each child or teenage behavior problem case is different, even if the adolescent behavior problem is the same. That’s because each individual child with a behavior problem is different. At SageWalk, we help the individual as well as the child or teenage behavior problem.
Acceptance of a child with a behavior problem into the SageWalk program for troubled teen youth is based upon several factors: current behavior and problems, behavioral and medical history, psychological information, and school history. Our admissions process is designed to be as simple as possible, without sacrificing the detail that is necessary to determine the appropriateness of SageWalk to treat your children with ADHD or your teenager’s adolescent behavior problem. All potential admissions are initially reviewed by admissions personnel, and assessed by our clinical team.
The first step in the admissions process is to determine whether your child is exhibiting the behaviors of a typical child or one of our teenage behavior problem cases at SageWalk. Please take a few moments to review this list. Is SageWalk Right for My Child?

Then take a moment to review our list of exclusions. If your child has any of the problems on this list, we are not the appropriate program for your child. List of Exclusions
The
next step in this process is to complete the application for enrollment. All questions must be answered before the clinical team will
accept an application for review. You may either fill the form out online or print out a copy and mail it in.

The next step is to complete the following forms which need original signatures. Please print them, fill them out, and mail it to:

Parents will then be asked to submit further information, such as a copy of a psychological evaluation/testing, or school records, to address any specific adolescent development and behavioral or educational concerns. Keep in mind that we want to insure that your child will benefit from our program. Typically, once we have received sufficient information, an enrollment decision can be made within 24-hours.
A
physical health examination will be administered upon arrival
to SageWalk. If you have concerns regarding your child's ability
to pass the required physical exam, you may have one done
by your physician before admittance. However, it is a SageWalk
requirement to have one performed by our approved physician
upon arrival, in addition to the one your physician has completed.
If
you need financial assistance, we have arrangements
with two academic loan programs.
You can contact them and apply either online or via telephone.
Prepgate Educational Loans tend to offer a slightly lower
interest rate. Achiever Loans tend to be a little easier to
qualify for. Please let them know that you are working with
us directly and they will speed up the application process.

Insurance Coverage
Although SageWalk does not work directly with insurance companies, most mental health plans do cover portions of the therapeutic and/or chemical dependency portions of the SageWalk program. Parents or guardians are solely responsible for pursuing reimbursement through his/her provider. Upon graduation, SageWalk can provide an itemized therapeutic billing statement with the diagnostic codes that insurance companies require. If you have any questions regarding insurance coverage, please contact your provider directly.
